Patient information has to be shared amongst healthcare professionals in a clear, effective and timely manner in order to devise an appropriate care plan and prevent adverse or fatal events. The healthcare setting is a dynamic environment with various healthcare professionals, diverse healthcare teams and multiple clinical data collected from both in-patients and ambulatory patients.
